Registered patients aged 16-74 who have not been seen for 3 years may request a consultation

Registered patients aged over 75 years who have not been seen in the previous 12 months may request a consultation. If you are unable to attend the surgery for this consultation because of your medical condition, a home visit may be arranged.

To register as a patient with our practice you should attend the surgery to collect a practice questionairre and registration form.

Patients may request to see a specific doctor at the practice when booking an appointment, however you should note that this may delay you being seen as soon as if you do not specify any one particular practitioner.
